• Filling the online form 8, designed by the Transplant of Human Organ Act 1994
• Uploading your passport sized photograph
• Pressing the submit button
• You will instantly receive a form with your details which you will take a printout of along with your donor card
• Cut the donor card and fit it in your wallet so that it is on your person all the time
• Ensure that the form you have taken is signed by two known witnesses, preferably family members with their contact details
• Scan and attach this form on the website after you log in with your id number and password
• When you register it is important that you tell those closest to you about your decision.
• Your voluntary donation will be acknowledged as soon as you submit your details with your photograph by the generation of your ID number. However, you will only be an active donor once you have submitted the form with details of your known witnesses. The Indian Transplant of Human Organ Act, 1994 acknowledges a signed form with known witnesses as a legal document of voluntary organ donation.
